Treeside Charter School Data Collection Notice
Treeside Charter School recognizes that the privacy and security of student personally identifiable
information is paramount and that we play a crucial role in providing that security. Our school’s
technology partners are dedicated to stringent regulations regarding the management of student data.
Treeside Charter School recognizes that student data belongs to the student and that the student or
parent/legal guardian can access that data at their request. Treeside Charter School has a high level of
commitment to managing student data responsibly and in accordance with all applicable state and
federal laws.
Below are brief descriptions of each of Treeside Charter School’s major data collections. The collection,
use, and sharing of student data have both benefits and risks. Parents and students should learn about
these benefits and risks and make choices regarding student data accordingly. We may not collect a
student’s social security number or, except as required in Section 78A-6-112, criminal record.

OPTIONAL STUDENT DATA
Treeside Charter School also collects information for optional programs and services. In order for the
district to collect this information, parents must sign a consent form that outlines what information is to
be collected, who is collecting the information, how it is to be stored and maintained, and how it is to be
used. If a parent opts out of sharing this information, they also opt out of the program or service
connected to that information.
HOW STUDENT DATA ARE STORED AND PROTECTED
TCS shall use reasonable data industry best practices to maintain and protect student data and other
education-related data, including teacher and employee data. Treeside Charter School maintains a
variety of agency policies that address data and information privacy which are intended to secure all

media containing sensitive or confidential data. Treeside faculty members who have access to student
data receive an annual student data privacy training as per state regulation.
Should the TCS contract with a third-party provider to collect, maintain, and have access to student
data, TCS shall ensure that the contract with the provider includes a provision that the data are the
property of the student and that the data will not be redisclosed with the student’s consent.
TCS will ultimately monitor and maintain control of the data. All data will be maintained in accordance
with TCS Information Technology Systems Security Plan, as described in R277-487-2(11).

STUDENTS’ AND PARENTS’ RIGHTS STATEMENT
Students own their personally identifiable student data. An educational entity shall allow the following
individuals to access a student’s student data that is maintained by the education entity: the student’s
parent; the student in accordance with the education entity’s internal policy and in the absence of a
parent, an individual acting as a parent to the student with legal documentation.
DATA BREACH
If a significant data breach occurs at an education entity, they shall notify the student’s parent or legal
guardian.
