This form notifies teachers of an extended leave and to facilitate coordination of missed work. This form must be completed BEFORE the student is absent. If approved, this allows the student to avoid attendance penalty.

Parents and students accepts responsibility for getting materials, completing assignments, and making arrangements for any testing prior to or upon return of absence as required by teacher(s). Parents and students realizes that any absence may adversely affect grades due to missed class time, etc.
